I've only seen the more conventional "disturbing" movies and none of what I would consider to be the more hardcore films, but the ones that stuck with me after watching are...

The Exorcist (agree with the comments by an earlier poster on it)
Requiem for a Dream (agreed with Karamazov - well made but so depressing that I don't want to see it again)
Wolf Creek (Director's Cut) - This one bothered me for a long while after watching
Hostel
Texas Chainsaw Massacre (original -- though the remake was surprisingly good, too, IMO)
Eden Lake
Funny Games
A Clockwork Orange (Mostly the first time I saw it around 18... as I've gotten older it hasn't retained the gut punch power it had on me in earlier years.)

Those are the ones off the top of my head that I can remember sort of pondering afterwards or having trouble sleeping, etc. Though it's not disturbing by today's standards at all, Friday the 13th Part III horrified the hell out of me as a kid. Jason's reveal to the protagonist he tried to (presumably) rape and kill years earlier, along with her dream sequence at the end of the movie really got under my skin. Probably the main reason the character has stuck with me all these years, despite most of the movies being absolute dross.

seen most of the films, but sometimes films which show more realism leaves me disturbed more than horror and such films,, watched a tamil film from India,,Naan Kadavul,,,
it shows the life of deformed people living as beggars and a story of a swami with whom their lives intervene,, feel depressed and distrubed as well in the end,
suggest u guys watch it,, u can compare it to Cannibal Holocaust probably in sense of realism
its second on my list of most disturbing films after Martyrs
https://www.youtube.com/watch?v=_SCYswKHBY8
what makes this film standout, is the film was made in the place where these deformed people actually live and the director made them act as well including kids , apart from the lead actor and actress,
Im actually surprised this film wasnt banned, probably will be banned in most countries

By far and away the MOST disturbing movie By FAR and I mean FAR........american guinea pig bouquet of guts and gore.............I so wish I could un-watch it. Some effects are iffy but the person who made that film has some serious problems. Do your self and don't watch it till the end.:yuck

Casino when Joe Pesci and his brother get beaten to death with bats, stripped babbling from the severe head damage and thrown into a hole in the middle of no where.

I can handle exorcism, poltergeist, omen, human centipede, SAW crap, but when it feels like a reality situation, it gets to me. When I first saw that in theaters, I was shocked. Scorsese fooled me with the voice over, I had no idea he was going to get it. Those cracks to the head and the way their bodies swelled up from the bat hits just really got under my skin.

Same. In fact I know I've mentioned that one before.

It's one of the many reasons I just don't like Casino very much. Can't watch it.

Then there's Goodfellas, one of my favorite movies that I've seen more times than I can count. Billy Batts' beating and shanking doesn't even bug me. None of the deaths do.

If you guys can't handle Casino do not even attempt to watch that movie I just posted. I've seen a lot and I wish I didn't watch it.

I know what you mean. I've got one of those jail broken TV boxes and clicked on it because of the stupid name. Then continued to watch because I wasn't sure if it was real or not. Then couldn't shut it down because my mind was boggled how someone would make something like this. Then the end.......my hair nearly fell out. SICK PEOPLE IN THIS WORLD.

I know there was some somewhat sick stuff back in the 80's and what not, but what reasoning is there for some of the absolutely repulsive stuff that is around these days? Do some of these dip ****s really consider it art? I wonder if these kinds of movies actually make money or not.

The rape scene in the original Girl with the Dragon Tattoo is very disturbing to watch. I love the film and have watched it several times, but I do tend to skip that scene now.
