It looks like:
• Smaller, narrow shoulder body.
• Repainted and modified shoulder armor.
• Head repainted | Nose mod.
• Belt weathered and modified.
• Chest box lightly weathered.
That's all I can identify from the pictures. Except for the tiny body the mods look pretty decent.

If it's intended to be Chad Vader as opposed to Darth Vader, it actually looks pretty accurate to me.
