That's the Child's Play 2 end scene doll by Tenoch Art and is pretty spot on. One of the better customs out there. I have this doll and it's pretty damn accurate in likeness. However, deal with him directly through his Facebook page and you can probably get a better deal. Not sure if he's still making this version though. He was thinking of retiring it.
