Batman didn't have an unmasked portrait. Of course, there might be a COH but I doubt it unless its an unmasked portrait...

Punisher is packed at the top of the Styrofoam almost. It'll come down to package engineering

Both hulks are variants of the green one which has a swappable head.

So it looks like so far only Apoc has it for packaging purposes, meaning it's rather likely that Batgirl will have a swap out head.

Alex said that regardless of the green Hulk, the heads are swappable because of shipping or something, that's why in included those. However I get what you mean.
